Discharge Planner

We are seeking a skilled Discharge Planner to join our team at the Commonwealth of Virginia. As a Discharge Planner, you will play a critical role in ensuring seamless transitions from hospital to post-acute care settings. Your primary responsibility will be to provide patient-centered care and support timely discharge outcomes.
Key Responsibilities:- Develop and implement discharge plans that meet the unique needs of each patient.
- Collaborate with healthcare teams to ensure timely and effective discharge transitions.
- Provide education and support to patients and their families to promote self-management and independence.
- Coordinate with community resources and healthcare providers to ensure continuity of care.
- Document all interventions and communications in the patient's medical record.
- Bachelor's degree in a related field, such as social work or healthcare administration.
- One year of experience in healthcare or a related field.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- A competitive salary range of $35.82 hourly.
