We are seeking a highly skilled and experienced Medical Director to lead our Hospital at Home Program at Lee Health. As a key member of our leadership team, you will be responsible for designing and implementing the long-term vision and strategy for this innovative program.
Key Responsibilities- Develop and execute the operational model for the Hospital at Home Program, ensuring seamless integration with existing hospital-based care services.
- Collaborate with cross-functional teams to identify patients eligible for care through the program, improve access to home-based care, and maximize facility capacity.
- Lead the development of cost-effective care delivery models that maintain service and quality levels equivalent to traditional hospital-based inpatient settings.
- Provide clinical oversight and leadership to ensure the program meets quality, safety, and regulatory standards.
- Work closely with subject matter experts to develop and implement policies and procedures for Emergency, Medical, and Surgical care at home.
- Medical Degree (MD or DO) required.
- Minimum of 5 years of clinical practice after residency training and/or population health management experience.
- Licensed Physician in the State of Florida.
- Board Certified in applicable specialty or subspecialty.
- Administration experience strongly preferred, with experience working with cross-functional teams and projects.
