Job Summary
We're seeking a skilled Care Navigator to join our team at AndHealth. As a Care Navigator, you'll play a vital role in guiding patients through their journey, ensuring they receive the care needed to live a healthier life. You'll work closely with our Physicians, Nurses, Health Coaches, and Patient Services team to deliver a "wow" experience for our patients.
Key Responsibilities
As a Care Navigator, your key responsibilities will include:
- Preparing exam rooms for patient care, ensuring they're properly stocked and cleaned.
- Monitoring patient phone calls, receiving, and routing test results for provider review, and collecting data regarding patient medications.
- Rooming patients, checking vital signs, and assessing patient needs to communicate with providers.
- Assisting providers with patient procedures as directed.
- Communicating effectively with patients, families, coworkers, and outside entities.
- Completing routine forms for patients as needed.
- Ensuring chart documentation in the Electronic Health Record (EHR) is correctly completed in a timely manner.
- Performing lab tests as directed by providers, preparing specimens, operating lab equipment, and maintaining laboratory documentation in accordance with CLIA standards and practice-specific protocols.
- Reconciling billing from offsite labs by comparing daily lab logs with patient EHR.
- Scheduling offsite diagnostic tests and communicating effectively with patients.
- Facilitating referral processes for patients as needed.
- Performing front office duties as needed.
- Adhering to all community health center policies and procedures.
- Other responsibilities as needed based on business needs.
Requirements
To be successful in this role, you'll need:
- A Bachelor's degree (preferred).
- LPN licensure (required).
- RN licensure (preferred).
- American Heart Association Basic Life Support Provider (BLS) certification (obtained or willing to obtain).
