POSITION SUMMARY:
In the context of end-of-life care, this role is dedicated to delivering comprehensive psychosocial evaluations and ensuring high-quality patient support. The incumbent will demonstrate proficiency in assessment, treatment planning, and discharge coordination, while also fostering the problem-solving and coping abilities of individuals. Responsibilities include:
  • Coordinating and executing suitable resource referrals.
  • Conducting supportive interventions aligned with patient diagnoses.
  • Collaborating effectively within a multidisciplinary team environment.

ESSENTIAL QUALIFICATIONS:

Education: A Bachelor's degree in Social Work is mandatory, with a Master's degree being highly desirable.

Experience: A minimum of two years of relevant experience in the healthcare sector is preferred. Proficiency in Microsoft Office Suite and familiarity with general office equipment is essential. The candidate must exhibit strong attention to detail and the ability to manage a dynamic schedule independently. Exceptional communication and interpersonal skills are required, along with a commitment to maintaining confidentiality in accordance with HIPAA regulations.

Licensure: A valid Arkansas Social Work license in good standing is required.


SAFETY SENSITIVE DESIGNATION:
This position is classified as safety sensitive under Arkansas Act 593 pertaining to Medical Marijuana.
