Inpatient Case Manager RN

At LifeBridge Health, we are seeking a highly skilled and compassionate Inpatient Case Manager RN to join our team. As a key member of our care management team, you will play a vital role in ensuring the seamless transition of patients from hospital to home, while promoting optimal health outcomes and reducing healthcare costs.
Key Responsibilities:- Conduct thorough assessments of patients to identify their care coordination and discharge planning needs.
- Develop and implement individualized care plans that address the unique needs of each patient.
- Collaborate with the clinical team, social workers, and other healthcare professionals to ensure a comprehensive and coordinated approach to patient care.
- Facilitate communication between patients, families, and healthcare providers to ensure a smooth transition of care.
- Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal patient outcomes.
- BSN preferred; ADN required.
- 3-5 years of related experience in a healthcare setting.
- Current Maryland Registered Nurse License or intent to achieve MD licensure if out of state.
- Case Management certification preferred.
