Inpatient Case Manager RN

LifeBridge Health is seeking a skilled Inpatient Case Manager RN to join our team. As a key member of our care team, you will play a vital role in ensuring the seamless transition of patients from hospital to home.
Key Responsibilities:- Conduct thorough assessments of patients to identify their care needs and develop personalized discharge plans.
- Collaborate with the clinical team and medical providers to ensure timely and effective care coordination.
- Facilitate communication among healthcare providers, patients, and families to ensure a smooth transition of care.
- Identify and address potential barriers to discharge, such as social determinants of health and lack of community resources.
- Develop and implement strategies to improve patient outcomes, reduce readmissions, and enhance the overall patient experience.
- BSN preferred; ADN required
- 3-5 years of related experience
- Maryland Registered Nurse License/Intent to achieve MD licensure if out of state
- Case Mgmt cert preferred 3-5 yrs of hire for role
