About the Role

Penn State Health is seeking a skilled Medical Navigator to join our team. As a Medical Navigator, you will play a critical role in ensuring that patients receive seamless care throughout their journey.

Job Description

In this role, you will be responsible for facilitating patient navigation, coordinating care transitions, and serving as a clinical resource to scheduling coordinators.

Key Responsibilities
  • Assist patients in navigating the healthcare system upon discharge
  • Coordinate care transitions to ensure continuity of care
  • Serve as a clinical resource to scheduling coordinators
  • Identify alternative locations of care when necessary
Requirements

To be successful in this role, you must have a current RN license in the state of employment or hold a multistate RN license through the interstate Nurse Licensure Compact. Additionally, you must possess AHA BLS certification prior to the end of the orientation period.

Estimated Salary

The estimated salary for this position is $80,000-$110,000 per year, based on location and experience.
