About the Role

We are seeking a dedicated and compassionate Healthcare Case Manager to join our team at Penn State Health. As a Healthcare Case Manager, you will play a critical role in coordinating patient care, developing individualized plans, and advocating for patients and their families.

Key Responsibilities
  • Conduct comprehensive assessments of patients' medical, social, and emotional needs
  • Develop and implement individualized care plans that address patients' unique needs and goals
  • Collaborate with healthcare providers, social workers, and other stakeholders to ensure seamless care coordination
  • Advocate for patients and their families to ensure access to necessary resources and support
Requirements

To be successful as a Healthcare Case Manager, you will need:

  • A bachelor's degree in a related field, such as nursing, social work, or healthcare administration
  • At least 2 years of experience in case management or a related field
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations
  • Excellent analytical and problem-solving skills, with the ability to navigate complex systems and prioritize tasks
