Job Summary

The Intake Care Coordinator plays a vital role in our organization, serving as the primary point of contact for new and returning clients. This position requires strong communication and interpersonal skills, as well as the ability to work collaboratively with a team to provide high-quality services.

Key Responsibilities
  • 40% Direct Client Care: Provide trauma-informed services to clients, primarily through phone and in-person interactions. Offer information on programs and services, assist with initial documentation, and deliver positive outcomes as measured by utilization of outcome measures.
  • 40% Coordination: Work with the Clinical Manager to manage the schedule of diagnostic assessments, prepare initial documentation, and coordinate services with clients, staff, and community partners.
  • 10% Documentation: Maintain accurate, concise, and concurrent documentation according to program procedures and policies, including client demographics and basic clinical information.
  • 5% Professional Development: Pursue professional development opportunities consistent with licensure and credential requirements to enhance personal and organizational growth.
  • 5% Miscellaneous Programmatic Duties and Intra-Organizational Support: Contribute to programmatic needs across the organization, participate in supervision and team meetings, and provide case management and/or crisis intervention/support as needed.
