About the Role

We are seeking a highly skilled and compassionate LPN Medication Care Manager to join our team at Sunrise Senior Living. As a key member of our care team, you will play a vital role in ensuring the highest quality of care and services for our residents.

Responsibilities
  • Medication Administration: Safely and accurately administer medications to residents in accordance with their individualized care plans.
  • Documentation: Maintain accurate and up-to-date records of medication administration, including resident charts and medication logs.
  • Resident Care: Provide direct care to residents, including assistance with daily living activities, as needed.
  • Quality Assurance: Participate in quality assurance activities, including audits and reviews, to ensure compliance with regulatory standards and Sunrise Senior Living policies.
Qualifications
  • Licensure: Current LPN licensure in good standing.
  • Experience: Minimum of six (6) months experience working with seniors in assisted living, home health, independent living, hospital, or long-term care environment.
  • Skills: Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare teams.
  • Education: High school diploma or equivalent required.
