Job Summary:
As a Nurse Care Specialist, you will be responsible for setting up services in patients' homes, making home visits, and performing assigned duties, including administration of medication, wound care, treatments, and procedures. You will also monitor patients' conditions, report changes to Clinical or Client Services Manager, and follow up with doctors' orders.

Responsibilities:
Set up services in patients' homes using Oasis
Make home visits to clients in designated geographic territories
Perform assigned duties, including administration of medication, wound care, treatments, and procedures
Monitor patients' conditions; reporting changes to Clinical or Client Services Manager
Follow up with, execute, and properly document doctors' orders
Perform client assessments as necessary
Case management and coordination

Qualifications:
A current license as a Registered Nurse in DE
Graduation from an accredited program
