Job Title: Medical Care Coordinator

WeCare Centers is seeking a dedicated Licensed Practical Nurse to join our team at Foltsbrook in Herkimer, NY. As a Medical Care Coordinator, you will play a vital role in ensuring the general health and well-being of our residents by providing high-quality nursing care.

Responsibilities:
  • Conduct thorough assessments of each patient's medical status upon admissions and throughout the course of treatment.
  • Provide regular patient status updates.
  • Assist in implementing patient's treatment plan under direction of the Registered Nurse.
  • Complete required documentation in an accurate and timely manner.
  • Supervise, administer and or assist with medications.
  • Provide direct care of residents as necessary.
  • Follow the plan of care for each resident.
  • Ensure the residents care standards are maintained.

Requirements:
  • Valid NYSED LPN Certification
  • Basic Life Support – American Heart Association (BLS)

Compensation: $60,000 per year (salary may vary based on experience)
