Job Title: Registered Nurse - Home Health

VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our Home Health team in Cedar Grove, New Jersey.

Job Summary:

We are looking for a dedicated and experienced RN to provide high-quality nursing care to patients in their homes. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities:
  • Assess and evaluate the needs of patients with multiple disease processes and co-morbidities
  • Develop and implement individualized plans of care to meet the unique needs of each patient
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and education
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Document all changes in patient condition and update care plans as needed
Requirements:
  • RN with Bachelor's degree in Nursing preferred
  • 1+ year of supervised clinical experience as a registered nurse
  • Excellent verbal and written communication skills
  • Strong computer skills
  • Excellent organizational and interpersonal skills
