Job Title: RN Case Manager

Humanity Hospice, LLC is seeking a skilled RN Case Manager to join our team. As a RN Case Manager, you will play a vital role in providing high-quality patient care and coordinating services to meet the needs of our patients and their families.

Job Summary:

The RN Case Manager will be responsible for assessing patient needs, developing and implementing individualized care plans, and coordinating services to ensure seamless care delivery. This is a full-time position that requires a strong commitment to patient-centered care and a passion for working in a dynamic, interdisciplinary team environment.

Responsibilities:
  • Conduct comprehensive assessments of patients and families to determine care needs
  • Develop and implement individualized care plans that address patient needs and goals
  • Coordinate services with healthcare providers, community resources, and other stakeholders to ensure seamless care delivery
  • Provide education and support to patients and families to promote self-care and independence
  • Collaborate with the interdisciplinary team to ensure comprehensive care and address patient needs
  • Document patient information and care plans in a timely and accurate manner
Requirements:
  • Graduate of an accredited school of nursing
  • Current licensure in the state of Oklahoma
  • One to two years of recent acute care experience in an institutional setting
  • One to two years of recent experience in home care
  • CPR certification
