RN Case Manager

Humanity Hospice, a leading provider of hospice care, is seeking a skilled RN Case Manager to join our team. As a RN Case Manager, you will play a vital role in delivering high-quality patient care and coordinating services to meet the needs of our patients and their families.
Job Summary:The RN Case Manager will be responsible for assessing patient needs, developing and implementing individualized care plans, and providing education and support to patients and their families. This is a full-time position that requires excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team.
Responsibilities:- Conduct comprehensive assessments of patients and their families to determine their needs and develop individualized care plans.
- Provide education and support to patients and their families on disease management, symptom control, and end-of-life care.
- Coordinate services with other healthcare providers and community resources to ensure seamless care delivery.
- Monitor patient progress and adjust care plans as needed.
- Collaborate with the interdisciplinary team to ensure comprehensive care and support.
- Graduate of an accredited school of nursing.
- Current licensure in the state of Oklahoma.
- One to two years of recent acute care experience in an institutional setting, and one to two years of recent experience in home care.
- ADN/Bachelor's degree, with one year of hospice care experience preferred.
