Key Responsibilities:

  • Adhere to the individualized care plan established by the hospice nursing team.
  • Engage in the coordination of comprehensive patient care, following established professional standards.
  • Execute healthcare tasks under the supervision of the hospice nurse, including vital sign monitoring and personal hygiene assistance.
  • Facilitate physical care for patients, including bathing, grooming, and mobility support.
  • Assist patients with exercises as directed by healthcare professionals.
  • Maintain a safe environment for patients, ensuring proper use of medical equipment.
  • Document patient activities and changes in condition for nursing staff review.
  • Provide respite for family caregivers during scheduled shifts.
  • Participate in team meetings and training sessions as required.

Qualifications:

  • Completion of a state-approved home health aide training program.
  • Certification as a home health aide with ongoing competency evaluations.
  • Basic first aid knowledge and understanding of hospice care principles.
  • Ability to work collaboratively and maintain confidentiality.
  • Proficient in computer and email usage.
