Job Summary

Kaiser Permanente is seeking a compassionate and skilled Home Health Aide to join our team. As a Home Health Aide, you will provide direct patient care under the supervision of a case manager, ensuring the highest level of comfort, dignity, and well-being for our patients.

This role requires a strong commitment to delivering personalized care, working collaboratively with healthcare professionals, and communicating effectively with patients and their families.

As a Home Health Aide at Kaiser Permanente, you will have the opportunity to make a meaningful difference in the lives of our patients and contribute to the delivery of high-quality, patient-centered care.

Key Responsibilities

Provision of Care

  • Assist patients with personal hygiene, including skin care, mouth care, and bathing
  • Assist patients with mobility, including transferring and ambulating
  • Assist with prescribed exercises, as taught by healthcare professionals
  • Prepare meals, including therapeutic diets, and assist patients with eating
  • Assist patients with bathroom needs, including commodes, bedpans, and urinals
  • Perform household services to support patients' self-care and prevent institutionalization
  • Assist patients with medication administration, under supervision
  • Perform other activities, as taught by healthcare professionals, to support patient care
  • Report changes in patient condition and needs to the supervising nurse or therapist
  • Document all care provided, including services and communication with healthcare professionals

Documentation and Communication

  • Prepare accurate and timely documentation of all care provided
  • Document all communication with healthcare professionals
  • Report changes in patient status to the case manager
  • Submit schedule logs, changes in schedule, and daily progress notes within agency guidelines
  • Participate in care coordination meetings

Planning, Organizing, and Productivity

  • Organize and prioritize tasks, ensuring completion within agency guidelines
  • Function independently, with supervision by the case manager as needed
  • Complete expense reports, time cards, and schedule logs
  • Demonstrate efficient use of supplies

Professional Behavior

  • Behave in a courteous and professional manner with patients, healthcare professionals, and maintain patient confidentiality
  • Participate in Departmental Quality Assurance activities and Utilization Review activities, as requested
  • Perform related duties, as required

Requirements
Basic Qualifications:
Successful completion of a training program approved by the Department of Health Services, pursuant to applicable federal and state regulations. Must be willing to work in a Labor Management Partnership environment.
Education:

  • High School Diploma/GED; California Home Health Aide Certificate
  • Current BLS certification; California Driver's License
License, Certification, Registration:
  • Driver's License (California)
  • Home Health Aide Certificate (California)
  • Basic Life Support
Estimated Salary: $65,000 - $85,000 per year, depending on experience and qualifications. This estimate is based on national averages and may vary depending on location and other factors.
