Job Summary:

The Certified Home Health Aide (CHHA) provides direct patient care under the supervision of a case manager, ensuring the highest level of care and support for patients in their homes.

Key Responsibilities:

Provision of Care

  • Assist patients with personal hygiene, including skin, mouth, hair care, and bathing.
  • Assist patients in and out of bed and facilitate ambulation.
  • Assist with prescribed exercises taught by health personnel.
  • Prepare meals, including therapeutic diets, and assist patients with eating.
  • Assist patients to the bathroom or with the use of commodes, bedpans, or urinals.
  • Perform household services to facilitate patients' self-care at home and prevent institutionalization.
  • Assist patients with medications, which are ordinarily self-administered.
  • Perform other activities taught by health professionals for specific patients, such as changing colostomy bags, non-sterile dressing changes, vital sign taking, and bowel and bladder hygiene care.
  • Report changes in patients' conditions and needs to the supervising nurse or therapist.
  • Complete records regarding services performed.
  • Take actions in emergent patient situations and immediately report to the supervisor.Documentation and Communication
    • Prepare timely, legible, and complete documentation of all care provided.
    • Document all communication with other providers.
    • Report stated or observed changes in patient status to the case manager.
    • Submit schedule logs, changes in schedule, and daily progress notes within the expected time frame per agency policy.
    • Participate in care coordination meetings.

    Planning, Organizing, and Productivity

    • Organize, plan, set priorities, and complete assignments within agency guidelines.
    • Function independently with supervision by the case manager for each patient every 14 days as needed.
    • Complete expense reports, time cards, and schedule logs.
    • Demonstrate utilization of supplies.

    Professional Behavior

    • Behave in a courteous, professional manner with patients, health professionals, and maintain patient confidentiality.
    • Participate in Departmental Quality Assurance activities and Utilization Review activities as requested.
    • Perform related duties as required.

    Supervisory Responsibilities: This job has no supervisory responsibilities.

    Grade 165

    Basic Qualifications: Experience
    Successful completion of a training program approved by the Department of Health Services pursuant to applicable federal and state regulation. Must be willing to work in a Labor Management Partnership environment.
    Education
    • High School Diploma/GED; California Home Health Aide Certificate. Current BLS certification; California Drivers License.

    • Computer literate preferred. Experience in automated clinical information systems preferred.
