Job Summary

The Case Manager will work as part of an interdisciplinary team to engage and provide services to homeless individuals in high-density areas of Santa Monica. The ideal candidate will have a strong understanding of the issues faced by the homeless population and be able to provide referrals, linkages, and coordinate care to support clients in achieving permanent housing.

Key Responsibilities:

  • Work with clients to identify and address barriers to permanent housing
  • Provide case management and aftercare services to clients placed in permanent housing
  • Develop and maintain relationships with community partners to ensure seamless referrals and services
  • Maintain accurate electronic records and statistical information
  • Attend team meetings, trainings, and retreats as required

Requirements:

  • Bachelor's degree or equivalent experience
  • Spanish speaking preferred
  • Knowledge of social services agencies in West LA and City of Santa Monica agencies serving the homeless
  • Excellent human relations and communication skills
  • Ability to work as part of an interdisciplinary team
