Job Description

Mobile Primary Care is seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our team of professionals in providing comprehensive medical care to geriatric patients.

Key Responsibilities:

  • Patient Care: Conduct thorough assessments, develop care plans, and perform routine follow-up evaluations for patients in the rehab and long-term care units.
  • Collaboration: Work closely with attending physicians, nursing staff, physical therapists, and other healthcare professionals to coordinate comprehensive patient care plans.
  • Documentation and Compliance: Maintain accurate and up-to-date medical records, ensuring compliance with facility policies, state regulations, and billing requirements.

Requirements:

  • Current licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) in New York.
  • Certification as an NP or PA from an accredited program.
  • Previous experience in geriatrics & primary care.
  • Strong clinical assessment skills and the ability to make autonomous clinical decisions.
  • Excellent communication skills and the ability to collaborate effectively within a multidisciplinary team.
