Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our team at InsideHigherEd. As a Case Manager, you will play a critical role in providing case management services to clients with multiple medical and psychosocial needs.

Key Responsibilities
  • Case Management Services: Provide case management services to clients, including psychosocial assessments, care planning, and coordination of services.
  • Client Advocacy: Act as an advocate for clients, connecting them with community-based organizations and public services to reduce barriers and enhance access to care.
  • Communication and Collaboration: Communicate effectively with clients, medical providers, and other stakeholders to ensure seamless care coordination.
  • Documentation and Reporting: Maintain accurate and timely documentation of client interactions, services provided, and outcomes.
  • Professional Development: Participate in ongoing training, clinical supervision, and professional development opportunities to enhance skills and knowledge.
Requirements
  • Education: Bachelor's degree in Social Work, Psychology, or a related field.
  • Experience: One year of related human service experience, preferably in case management.
  • Skills: Strong organizational skills, attention to detail, and ability to work with diverse populations.
Preferred Qualifications
  • Experience: Three years of case management experience in a healthcare or public welfare setting, with knowledge of HIV, substance abuse, and psychiatric illness.
Special Knowledge, Skills, and Abilities
  • HIV Disease Knowledge: Demonstrate general knowledge of HIV disease, treatment, and resources.
  • Public Entitlement Programs: Knowledge specific to public entitlement programs related to healthcare.
  • Cultural Competence: Able to work with culturally diverse populations.
  • Organizational Skills: Strong organizational skills with attention to detail and follow-through.
