Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in Green Bay. As a Private Duty LPN, you will provide high-quality clinical services to patients in their homes, working under the supervision of a Registered Nurse.

Responsibilities
  • Provide direct nursing care to patients as prescribed by physicians, including administering medications and treatments.
  • Record clinical and progress notes, as well as coordinate other patient care activities and services.
  • Work collaboratively with the healthcare team to ensure the highest quality of patient care.
  • Document patient visits thoroughly and accurately, following regulatory and payer requirements.
  • Participate in the agency's care coordination process, including timely reporting of patient needs and changes in condition.
Requirements
  • Current LPN licensure in the state of practice.
  • A minimum of one year of acute care experience, with previous home care experience preferred.
  • CPR certification.
  • Ability to relate to and treat acutely ill/chronically ill patients and their families.
  • Strong patient management and documentation skills.
