ROLE SUMMARY

Supports the Director of MDS in the formulation, execution, and submission of resident assessments in compliance with state regulations and the facility's policies and objectives.

RESPONSIBILITIES

• Ensures that all residents receive fair treatment, kindness, dignity, and respect, while safeguarding their rights at all times.
• Respects and honors all residents' personal and property rights during the execution of duties, including proper etiquette before entering a resident's room.
• Adheres to all organizational and departmental policies and procedures.
• Participates in the development and completion of initial and comprehensive resident assessments (MDS) and Care Plans in accordance with current regulations and guidelines, including the implementation of CAAs and Triggers and Plan of Care.
• Ensures that care plans incorporate measurable objectives and timelines to address residents' medical, nursing, mental, and psychosocial needs, involving appropriate health professionals in the assessment process, and developing required activity schedules.

KNOWLEDGE, SKILLS, AND QUALIFICATIONS

• 1-3 years of experience providing direct care to multiple patients/residents, with at least one year of prior MDS experience in a Skilled Nursing environment preferred.
• Nursing degree from an accredited institution or graduation from an approved LVN program.
• Must hold a current, unencumbered license to practice as an LVN in California.
• Current valid CPR certification.
• Familiarity with clinical standards of practice, regulations, and reimbursement policies governing long-term care and Medicare participation.

Salary Range: Starting at $40.00/hr for LVNs
