Job Summary:

  • The LPN Consultant is tasked with overseeing the delivery of Wellness Coordination services as stipulated by the relevant Indiana Code.
  • Must be prepared to conduct wellness services across designated counties.
  • This role involves facilitating pre-service and in-service training sessions in CPR/First Aid and Wellness strategies.
  • The LPN Consultant will deliver Wellness Coordination services, ensuring the health status, high-risk plans, and medication administration for program participants are effectively monitored. They will serve as a vital resource for the entire service team regarding wellness plans.

Qualifications:

  • Current Indiana nursing license as an LPN is required.
  • A minimum of two years of experience working with individuals with disabilities is preferred.
  • Prior supervisory or training experience is advantageous.
  • Strong verbal and written communication skills are essential.
  • Ability to effectively communicate, present, and prepare concise reports for various employee levels.
  • Certification from the Red Cross to instruct CPR/First Aid is mandatory.
  • Dependable transportation, a valid Indiana driver's license, and proof of personal automobile insurance are necessary.
  • Capability to lift up to 35 pounds; upper body strength is required.
  • Willingness to travel between offices is essential.
  • Flexibility to work non-standard hours is expected.
