Position Overview:
As a Service Coordinator II, you will play a crucial role in providing individualized support and advocacy for children with developmental disabilities and their families. Your expertise will help ensure that these individuals receive the necessary resources and services tailored to their unique needs.

Key Responsibilities:
  • Maintain ongoing communication with individuals and families, conducting Individual Family Service Plans (IFSP) and Individual Program Plans (IPP).
  • Engage in Interdisciplinary Team (IDT) meetings and collaborate with community stakeholders, including educational institutions and day programs.
Assist families in accessing community resources such as:

  • Local schools, recreational activities, and support groups.
  • Employment and habilitation programs.
  • Government agencies (e.g., Social Security, Medi-Cal).
  • Specialist services (e.g., speech therapy, occupational therapy).
Service Coordination:
Facilitate and manage TCRC-funded services, which may include:
  • Behavioral services and independent living training.
  • Respite care options, both in-home and out-of-home.
  • Day programs for infants and adults.
  • Durable medical equipment and specialized services.
Essential Functions:
  • Conduct assessments that highlight the strengths and challenges faced by families.
  • Involve families in all aspects of service planning and support.
  • Document and monitor the implementation of services and supports effectively.
  • Build and maintain relationships with community vendors and agencies.
  • Provide training and information regarding developmental disabilities and available resources.
  • Utilize strong communication skills to foster respectful interactions with individuals and families.
Qualifications:
A Bachelor's degree in Human Services or a related field is required, along with a minimum of two years of experience in social or mental health services. A valid driver's license and reliable transportation are essential for this role. Experience with developmental disabilities is preferred but not mandatory.

Physical Requirements:
The role requires manual dexterity for office equipment operation, the ability to sit for extended periods, and travel to various locations as needed. Physical tasks may include lifting light objects and navigating uneven terrain.
