Detailed Job Description:
Deliver patient care in alignment with established practice standards. Enhance team patient care access by providing direct patient services through both in-person and virtual consultations, while collaborating with fellow providers to address patient needs between visits.
Main Duties:
- Collaborate with assigned healthcare providers within a clinic to ensure optimal patient care
- Engage in the healthcare team.
- Foster communication within the team and with other interdisciplinary healthcare staff.
- Serve as a resource for team members.
- Collaborate with the primary care team for program development.
- Act as a liaison with external agencies as necessary.
- Provide clinical care for patients
- Conduct comprehensive patient assessments.
- Determine diagnoses and treatment plans for illnesses.
- Order and interpret medical diagnostics in conjunction with the attending physician or advanced practice clinician.
- Prescribe medications, treatments, therapies, and medical equipment.
- Conduct patient education to manage chronic conditions or promote wellness.
- Refer patients for specialty care as needed.
- Consult with physicians or more experienced advanced practice clinicians when patient needs exceed scope of practice or when treatment plans are unclear.
- Perform additional duties as assigned.
Required Qualifications:
- Completion of a Master's or PhD Nurse Practitioner or Physician Assistant program.
- New graduates are encouraged to apply.
Preferred Qualifications:
- One year of experience in Primary Care as a Nurse Practitioner or Physician Assistant.
Licenses/Certifications:
- Registered Nurse (for Nurse Practitioners) license in the State of MN and/or WI required.
- Nurse Practitioner or Physician Assistant license in the State of MN and/or WI required.
- DEA certification required.
- BLS certification required within 180 days.
Physical Demands:
Medium Work:
Lifting weight up to 32 lbs. occasionally (*Allina Safe Patient Moving Policy), up to 25 lbs. frequently.
