Home Health Physical Therapist

LHC Group is seeking a skilled Physical Therapist to join our team in Tallahassee, Florida.
About the Role:We are looking for a compassionate and dedicated Physical Therapist to provide high-quality care to our patients in the comfort of their own homes. As a Physical Therapist with LHC Group, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Responsibilities:- Evaluate and assess patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.
- Develop and implement individualized treatment plans to improve and enhance patient well-being.
- Provide services within the scope of practice as defined by state laws governing the practice of physical therapy, in accordance with the plan of care, and in coordination with other members of the healthcare team.
- Observe and report patient response to treatment and any changes in condition to the nurse supervisor and/or physician.
- Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice and evidence-based techniques.
- Instruct and inform participating members of the healthcare team, the patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
- Current Physical Therapy licensure in the state of Florida.
- Current CPR certification.
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
