Key Responsibilities
  • Conduct thorough evaluations and assessments to identify patients' needs and develop individualized care plans.
  • Provide evaluative, consultative, and treatment interventions to patients in their place of residence, as referred by a licensed physician.
  • Maintain effective communication with patients, their families, and other internal and external customers as appropriate.
  • Document all interactions, assessments, and treatments accurately and in a timely manner, in accordance with policy and regulatory requirements.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and patient outcomes.
  • Stay up-to-date with industry developments, best practices, and regulatory requirements to provide evidence-based care.
Requirements
  • Valid professional license in the state of practice.
  • Graduate of an accredited Physical Therapy Program/ACOTE School/ASHA School.
