We are seeking a skilled Licensed Practical Nurse to join our team at ResCare Community Living. As a LPN, you will play a vital role in providing high-quality patient care and support to individuals with intellectual, developmental, or cognitive disabilities.
Job Summary:The Licensed Practical Nurse will be responsible for planning and managing patient care according to each individual's needs. This includes interviewing patients, recording their medical history and physical condition, and obtaining vital signs. The LPN will also provide routine care for patients, including medication administration, feedings, and oxygen therapy as needed.
Responsibilities:- Provide 1:1 care for clients, following their plan of care and maintaining their dignity and independence.
- Monitor clients' vital statistics and report abnormal findings to the case manager and physician.
- Perform medication administration, feedings, oxygen therapy, and ostomy care as needed, and provide assessments of results.
- Complete routines established by other care professionals, such as Physical Therapist, Occupational Therapist, and Speech Therapist.
- Ensure infection control policies are maintained at all times and report possible communicable diseases or infections.
- Accurately document nursing actions and communicate with family, case manager, and physicians.
- Order supplies and medications as needed to ensure adequate amounts are on hand.
- Ensure all patient needs are met in accordance with physician instructions.
- Monitor incidents involving individuals and ensure incident reports are completed timely and accurately.
- Completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse.
- Knowledge of individual service plans, including understanding of client rights and health needs.
- Proof of current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice.
- CPR Certified.
- Excellent interpersonal skills, including customer service, patience, empathy, and compassion.
- In-depth understanding of patient care policies and regulations.
- Familiarity with symptoms and signs of improvement for several medical conditions.

Our community living services are provided in 27 states with several thousand programs and a comprehensive range of high-quality services.
