Key Responsibilities- Evaluate and provide speech and language therapy services to patients in their homes, utilizing evidence-based practices and techniques.
- Develop and implement individualized treatment plans, working closely with patients, families, and healthcare providers to ensure seamless care coordination.
- Conduct comprehensive assessments, including speech and language evaluations, to identify areas of need and develop targeted interventions.
- Provide education and training to patients and their families on effective communication strategies, alternative communication methods, and other relevant topics.
- Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure comprehensive care and optimal patient outcomes.
- Document patient progress, maintain accurate records, and submit timely reports to ensure compliance with regulatory requirements.
- Participate in ongoing professional development, staying current with best practices, research, and industry developments to enhance clinical expertise and patient care.
- Master's degree in Speech-Language Pathology or related field.
- Current certification in Speech-Language Pathology (CCC-SLP) or eligible for certification.
- Minimum of six months of experience as a speech therapist or speech language pathologist.
- Home health experience preferred.
- Current and unrestricted license to practice in the state of employment.
- Current CPR certification.
- Excellent communication and interpersonal skills, with ability to work effectively with diverse populations.
