We are seeking a highly skilled Speech Language Pathologist to join our team at CenterWell Home Health. As a key member of our care team, you will play a vital role in providing high-quality, patient-centered care to individuals in their homes.

Key Responsibilities
  • Evaluate and provide speech and language therapy services to patients in their homes, utilizing evidence-based practices and techniques.
  • Develop and implement individualized treatment plans, working closely with patients, families, and healthcare providers to ensure seamless care coordination.
  • Conduct comprehensive assessments, including speech and language evaluations, to identify areas of need and develop targeted interventions.
  • Provide education and training to patients and their families on effective communication strategies, alternative communication methods, and other relevant topics.
  • Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure comprehensive care and optimal patient outcomes.
  • Document patient progress, maintain accurate records, and submit timely reports to ensure compliance with regulatory requirements.
  • Participate in ongoing professional development, staying current with best practices, research, and industry developments to enhance clinical expertise and patient care.
Requirements
  • Master's degree in Speech-Language Pathology or related field.
  • Current certification in Speech-Language Pathology (CCC-SLP) or eligible for certification.
  • Minimum of six months of experience as a speech therapist or speech language pathologist.
  • Home health experience preferred.
  • Current and unrestricted license to practice in the state of employment.
  • Current CPR certification.
  • Excellent communication and interpersonal skills, with ability to work effectively with diverse populations.
