We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team at CVMC -APP Primary Care in Berlin, Vermont. The ideal candidate will provide high-quality patient care and work collaboratively with our healthcare team to deliver exceptional patient outcomes.
Responsibilities:
- Provide primary and specialty care to patients in our Express Care location
- Conduct thorough health assessments and develop individualized treatment plans
- Collaborate with our healthcare team to coordinate patient care and services
- Stay up-to-date with the latest medical research and best practices in primary care
Requirements:
- Current licensure as a Nurse Practitioner or Physician Assistant in Vermont
- Experience in primary care or a related field
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
