ACT Case Manager

We are seeking a highly skilled and compassionate ACT Case Manager to join our multidisciplinary team at CrossWinds Counseling & Wellness. As a key member of our team, you will provide intensive case management services to individuals with severe mental illness, working collaboratively with clients, families, and community resources to promote independent living and successful community integration.
Key Responsibilities- Attend and participate in ACT team meetings to review referrals, address issues, and report on client activities and progress.
- Provide direct case management services to adults through the ACT model, focusing on strengths-based approaches and individualized plans of care.
- Assist clients and their families in identifying, securing, and maintaining community resources, and coordinate referrals and services among agencies and organizations.
- Maintain accurate and complete records of client services, submitting daily progress notes, client status reports, and other documentation in a timely and acceptable format.
- Develop knowledge of community resources and access services for the benefit of clients, establishing and maintaining positive working relationships with clients, community partners, and agency staff.
- Associate Degree in a related field, with 2 years of experience in the health and human services field.
- One year of experience working with SPMI populations.
- Complete Case Management Training and provide training certificate to the agency within six months of hire.
- Ability to work with persons with severe and persistent mental illness, possess demonstrated interpersonal skills, and ability to react effectively in a variety of human service situations.
This role operates in both community and professional office settings, primarily working with clients out in the community, in their homes, in offices of other providers, and within residential facilities. The employee must occasionally lift or move up to 15 pounds and have specific vision abilities required by the job, including close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
TravelRegular travel within Lyon to provide case management services and may include surrounding counties as needed.
