About the Position

The Mental Health Support Specialist is responsible for supporting patients with their behavioral health needs. This position involves working closely with licensed behavioral health providers to coordinate referrals, support services, and track patient outcomes.

Key responsibilities include engaging with patients during warm hand-offs, offering education about integrated care, and assisting with scheduling for initial and follow-up visits. You will also facilitate patient engagement and follow-up care, track patient follow-up and clinical outcomes, and document encounters in the EHR.

A strong focus on patient-centered care and effective communication is essential in delivering high-quality patient care. The successful candidate will possess excellent interpersonal skills, be able to work effectively in a team environment, and demonstrate a commitment to delivering exceptional patient care.

We offer a competitive salary of $23.39 per hour, based on experience, as well as a comprehensive benefits package that includes paid time off, nine paid holidays, discounted healthcare, and matching retirement plan.
