About the Role

We are seeking a skilled RN Care Coordinator to join our team at Virginia Mason Franciscan Health. As a key member of our healthcare team, you will play a vital role in ensuring the seamless progression of care for our patients.

Key Responsibilities
  • Conduct thorough discharge planning assessments to identify patient needs and develop effective care plans.
  • Collaborate with multidisciplinary teams to ensure timely and smooth transitions to post-acute services.
  • Identify and mitigate risks for readmission, implementing interventions to promote optimal patient outcomes.
  • Deliver patient education and advocacy, ensuring patients and families are empowered to make informed decisions.
Requirements
  • Graduate of an accredited school of nursing.
  • Minimum two years of acute hospital clinical experience or a Master's degree in Case Management or Nursing field in lieu of 1 year experience.
  • Current licensure as a Registered Nurse in the state of Washington (RN-WA).
  • BLS required within 3 months of hiring if located within hospital.
Preferred Qualifications
  • Bachelor's Degree in Nursing (BSN) or related healthcare field.
  • At least five years of nursing experience.
  • Certified Case Manager (CCM), Accredited Case Manager (ACM-RN), or UM Certification preferred.
