Job Summary:

The Nurse Patient Navigator at VCU Health serves as a key member of the multidisciplinary team, providing comprehensive care and support to patients with cancer. This role requires a strong clinical background, excellent communication skills, and the ability to navigate complex healthcare systems.

Key Responsibilities:

Collaborate with patients, families, and healthcare providers to develop and implement individualized care plans
Provide education and support to patients and families on cancer diagnosis, treatment options, and self-care strategies
Facilitate communication between patients, families, and healthcare providers to ensure seamless care coordination
Identify and address social determinants of health that impact patient outcomes
Develop and maintain relationships with community resources and healthcare providers to ensure access to comprehensive care

Requirements:

Current RN licensure in Virginia or eligible/compact state
Minimum of three years of relevant clinical experience in nursing, patient education, case management, and/or navigation
Previous experience in specialty area and/or nurse navigation/leadership
Baccalaureate Degree in Nursing from an accredited School of Nursing

Preferred Qualifications:

Master's Degree in Nursing from an accredited School of Nursing
Previous nurse navigation or leadership experience
Experience within an academic, teaching hospital

Work Environment:

This role requires the ability to work in a fast-paced environment, prioritize multiple tasks, and maintain a high level of professionalism in all interactions. The Nurse Patient Navigator will be required to work primarily weekdays but may be expected to work into the evening or on the weekend, depending on assignment. May be required to go to all VCUHS locations.
