* Hold meetings with patients/residents, families, and significant others as requested to provide support and information to improve quality of life.
* Perform patient/resident discharge planning activities, including family/patient education, nursing center documentation, and referrals to appropriate agencies and suppliers.
* Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.
