Key Responsibilities
  • Provide in-home crisis-oriented treatment, education, and support to family units at risk of out-of-home placement.
  • Develop and implement individualized treatment plans to address the unique needs of each family.
  • Collaborate with referral sources, agencies, and institutions to ensure a comprehensive approach to family support.
  • Conduct regular case reviews and provide written reports to supervisors and stakeholders.
  • Participate in team meetings and contribute to the development of best practices in family preservation.
Requirements
  • Bachelor's degree in social work or a related field.
  • Minimum two years of experience working with multi-problem children and families.
  • Valid driver's license and reliable transportation.
  • Ability to work independently and as part of a team.
