We are seeking a skilled and compassionate Afterhours Registered Nurse to join our team at Assured Hospice of Port Angeles, a part of LHC Group.

As an Afterhours Registered Nurse, you will play a vital role in providing end-of-life care to patients and their families after regular business hours. Your primary focus will be to conduct comprehensive and timely delivery of hospice care, including triaging patient needs over the phone, responding to crisis calls, and making initial nursing evaluations.

Key Responsibilities:

  • Provide high-quality, patient-centered care to patients and their families after regular business hours.
  • Triage patient needs over the phone and respond to crisis calls with urgency and compassion.
  • Conduct initial nursing evaluations to determine eligibility for hospice services.
  • Perform afterhours visits, including admission visits, death visits, recertification visits, and routine visits.
  • Identify and address patient/family physical, psychosocial, emotional, and environmental needs.

Requirements:

  • One year of clinical experience as a Registered Nurse.
  • Current RN licensure in the state of practice.
  • Current CPR Certification.
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
