Job Title: Afterhours Registered Nurse Hospice

We are seeking a compassionate and skilled Afterhours Registered Nurse to join our team at Assured Hospice, a part of LHC Group. As an Afterhours Registered Nurse, you will play a vital role in providing end-of-life care to patients and their families after regular business hours.

Responsibilities:
  • Conduct comprehensive and timely delivery of hospice care for patients and their families after regular business hours.
  • Triage patient needs over the phone until a nurse can provide an in-person visit.
  • Respond to crisis calls from patients and families afterhours with urgency and empathy.
  • Make initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
  • Perform afterhours visits, including admission visits, death visits, recertification visits, and routine visits.
  • Identify and address the physical, psychosocial, emotional, and environmental needs of patients and their families.
Qualifications:
  • One year of clinical experience.
  • Current RN licensure in the state of practice and one year of clinical experience.
  • Current CPR Certification.
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle.
