Job Description

Role Overview

We're seeking a highly skilled Behavioral Health Care Coordinator to join our team. As a key member of our Behavioral Health Program, you will play a vital role in coordinating care for our members with behavioral health needs. Your primary responsibility will be to provide exceptional support and guidance to our members, ensuring they receive the best possible care and outcomes.

Key Responsibilities

  • Coordinate care for members with behavioral health needs, working closely with our clinical teams and community partners to ensure seamless transitions and high-quality care.
  • Provide empathetic and compassionate support to our members, addressing their unique needs and concerns.
  • Collaborate with our interdisciplinary team to identify high-risk patients, resource needs, and next steps, ensuring our members receive the best possible care.
  • Develop and maintain relationships with community behavioral health partners, including locating and confirming in-network providers and services.
  • Operate independently, be self-sufficient, and be willing to go the extra mile to ensure our members receive exceptional care.

Requirements

  • Bachelor's Degree and 3 years' work experience in a healthcare setting.
  • Two (2) years of experience in a behavioral health setting.
  • The ability to comfortably multi-task, listening, talking, and typing simultaneously.
  • Comfortable working with internal and external stakeholders to advocate for our patients.

Desired Skills and Experience

  • Prior experience with customer service.
  • Bilingual preferred (English/Spanish) but not required.
  • Geriatric experience or experience working with a Medicare population.
  • Health insurance experience, particularly MA or Medicaid.
  • Prior experience working with complex patients and/or underserved populations.
