We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Nurse. As a Home Health Nurse, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to develop and implement personalized care plans.

Key Responsibilities
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted care plans.
  • Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
  • Collaborate with patients' families and caregivers to educate them on care plans, medication management, and other essential aspects of patient care.
  • Communicate effectively with healthcare providers, insurance companies, and other stakeholders to ensure seamless care coordination.
  • Document patient progress, including notes, assessments, and care plans, to maintain accurate and up-to-date records.
Requirements
  • Active Registered Nurse (RN) License in Colorado.
  • Graduate of an accredited school or program of nursing.
  • At least 1200 hours of experience as an RN.
  • Healthcare CPR/BLS Certification (demonstration course required).
  • Home health care experience preferred.
  • Pass a background check and drug test (we are required to test for THC).
