Job Overview

We are seeking a highly motivated and compassionate Care Coordinator to join our team at Mental Health Partners. As a Care Coordinator, you will play a vital role in supporting clients with resource navigation, coordinating benefits and services, and working collaboratively with a team to ensure client needs are met.

This is a 50/50 office-based and community-based position, requiring strong communication and interpersonal skills. If you are passionate about helping clients achieve their health and wellness goals, we encourage you to apply.

Responsibilities
  • Manage a caseload of clients and their diverse needs
  • Collaborate with a team to ensure client needs are met
  • Provide resource navigation, benefit coordination, and service registration
  • Develop and maintain relationships with clients and community partners
  • Participate in ongoing training and professional development
Requirements
  • Bachelor's Degree in Psychology, Social Work, Counseling, or a related field
  • Experience working with adults in an outpatient setting, with a focus on mental illness and substance use issues
  • Knowledge of clinical modalities, including Motivational Interviewing and Dialectical Behavior Therapy
  • Strong communication and interpersonal skills
  • Ability to work collaboratively as part of a team
