Job Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Ormond Rehabilitation and Nursing Center. As a Staff Nurse (LPN/LVN), you will play a vital role in providing high-quality care to our residents.

Key Responsibilities
  • Direct the day-to-day functions of the Nursing Assistants, ensuring adherence to regulations and guidelines.
  • Assist in planning resident discharge plans, ensuring a smooth transition to post-acute care.
  • Complete accurate and informative nursing notes, reflecting the care provided and resident responses.
  • Administer medications as ordered by physicians, adhering to federal, state, and local regulations.
  • Order prescribed medications, supplies, and equipment, ensuring timely delivery.
  • Ensure accurate narcotic records and handle narcotics in accordance with regulations.
  • Make daily rounds to ensure nursing personnel are performing tasks in accordance with acceptable standards.
  • Notify attending physicians and next-of-kin of changes in resident conditions or incidents.
  • Participate in center in-service programs, staying up-to-date on best practices.
  • Complete daily required nursing evaluations and documentation in Point Click Care.
Requirements
  • Must hold a current and valid LPN license in the state employed.
  • Long-term care/post-acute care setting experience preferred.
