Palliative Care Specialist

Affinity Hospice is a reputable organization seeking an experienced Palliative Care Nurse Practitioner to join our team.
Job SummaryWe are offering a competitive salary of $120,000 per year for this full-time position. The successful candidate will be responsible for delivering high-quality palliative care services to patients in various settings.
Key Responsibilities:- Initiate palliative care consultations and develop appropriate treatment plans for patients
- Consult with medical colleagues and communicate effectively about patient care provided
- Provide health education, counseling, and referrals to other healthcare professionals and community resources as needed
- Prepare and maintain accurate patient records and charts to support sound medical practice and reimbursement for services provided
- Current unrestricted license to practice as a registered professional nurse in the state of employment
- Graduate of an approved Nurse Practitioner program
- Current unrestricted license to practice as a Nurse Practitioner by the Board of Nursing in the state of employment
- Current and continued CPR certification
- Certified Hospice and Palliative Care Nurse (CHPN) preferred
- Minimum of 2 years of palliative care and/or hospice experience required
