About the Role

We are seeking a highly skilled and compassionate Behavioral Health Counselor to join our team at Seven Hills Foundation. As a member of our CBHI home-based services team, you will provide therapeutic interventions to youth and adults, working collaboratively with an In-Home Therapist to support and assist with the implementation of the clinician's treatment plan.


Key Responsibilities

• Provide high-quality care to clients, adhering to best-practice standards and agency guidelines.
• Develop and maintain effective relationships with clients, families, and providers, promoting client-centered care and recognizing client voice and choice.
• Conduct thorough assessments, develop and implement individualized care plans, and provide ongoing support and coaching to clients and their caregivers.
• Collaborate with multidisciplinary teams to ensure comprehensive care and address complex client needs.
• Maintain accurate and timely documentation, adhering to agency policies and procedures.
• Participate in ongoing training and professional development to enhance skills and knowledge.


Requirements

• Master's degree in a relevant field (e.g., social work, counseling, psychology) and licensure as a mental health professional (e.g., LCSW, LMFT, LPCC).
• Experience working with youth and families, preferably in a home-based or community-based setting.
• Strong communication and interpersonal skills, with ability to work effectively with diverse populations and stakeholders.
• Ability to work independently and as part of a team, with strong problem-solving and critical thinking skills.
• Valid driver's license and reliable transportation, with ability to travel to client homes and community-based settings.
