Job Title: Licensed Practical Nurse

Sevita is a leading provider of home and community-based specialized healthcare services. We are seeking a skilled Licensed Practical Nurse (LPN) to join our team.

Job Summary:

The LPN will provide high-quality nursing care to individuals with developmental disabilities, acquired brain injuries, substance abuse issues, and/or emotional or behavioral challenges. The successful candidate will work collaboratively with our multidisciplinary team to develop and implement individualized care plans.

Key Responsibilities:
  • Provide basic care and medical treatment to individuals served
  • Complete health assessments and provide input for the development of nursing care plans
  • Follow-up on physician orders and ensure follow-through on medical appointments
  • Monitor, administer, and record medications promptly
  • Conduct quality assurance checks on healthcare records
  • Maintain confidentiality and respect human rights
  • Partner with external agencies to coordinate services and ensure consistent treatment
Requirements:
  • Associate Degree in Nursing or related field
  • One year of experience working with the population served in a nursing capacity
  • Current state LPN license and other certifications as required by state licensing regulations
  • Strong communication skills and ability to build rapport with others
  • Self-motivated and detail-oriented with ability to multi-task
