About the Role

As a key member of our community partnerships team, you will play a critical role in implementing our Permanent Supportive Housing (PSH) projects, focusing on financial security, physical security, social connections, and delivery of long-term community-based supportive services.

Key Responsibilities
  • Conduct Assertive Tenant Engagement: Encourage participation in services and responsiveness to direction or requests from property management, providing consistent forward-facing support to tenants, service providers, and property managers.
  • Coordinate On-Site Schedules and Amenities: Oversee weekly recreational and community building activities that support tenants in meeting the obligations of their housing tenancy and expectations of the community.
  • Coordinate Tenant/Community Meetings and Activities: Foster community engagement and proactively address residential challenges with tenants and property management.
  • Work with Partnering Agencies: Coordinate and host on-site services and activities/events with partnering agencies.
  • Provide Early Identification and Intervention: Identify and address tenant behaviors that may jeopardize housing, such as late rental payment and other lease violations, and share findings with property management and the service provider.
  • Coach and Assist Tenants: Assist tenants with processes needed to maintain housing stability, including lease renewal forms, connecting tenants with agencies to obtain supportive documentation for housing, and referring tenants to appropriate support services.
  • Anticipate and Ease Staff Transitions: Use a team approach to ensure that all tenants have relationships with more than one staff member.
  • Coach, Develop, and Maintain Key Relationships: Develop and maintain key relationships with property managers and service providers to maintain tenant housing stability and project safety.
  • Solicit and Maintain Data: Collect and maintain data on tenant participation, satisfaction, and requests for additional services and activities.
  • Provide Crisis Intervention and Support: Provide crisis intervention, general information, and referrals to all project tenants, as well as support tenants connecting to Medicaid and offer additional support to providers working with difficult-to-serve clients.
  • Provide Tenant Conflict Resolution and Moving On Strategies: Provide tenant conflict resolution and Moving On strategies when necessary.
  • Ensure Data Collection and Entry: Collect and enter all necessary tenant/project data at assigned properties, including Homeless Management Information System (HMIS) data entry on a monthly basis (as required by project).
  • Document Meetings and Communications: Document meetings/communications with service providers and property management and maintain accurate records for funders, compliance, etc.
  • Ensure Advocacy and Linkage: Ensure advocacy and linkage with community resources to address housing stability concerns and prevent eviction whenever possible.
Requirements
  • Strong Leadership and Interpersonal Skills: Possess a high degree of initiative and strong leadership and interpersonal skills.
  • Ability to Work in a Fast-Paced Environment: Ability to work in a fast-paced, flexible environment.
  • Understanding of Residential Settings: Understanding of residential settings and commitment to meeting housing and other basic needs.
  • Experience with Evidence-Based Practices: Experience with evidence-based practices such as harm reduction, motivational interviewing, critical time intervention, and Housing First principles.
  • Safe Driving Practices: Demonstrated safe driving practices and ability to follow directions.
  • Excellent Organizational and Analytical Skills: Excellent organizational and analytical skills.
  • Strong Communication Skills: Strong communication skills, both oral and written, with proven problem-solving abilities.
