Job Title: Home Health Speech Therapist

Advanced Home Health and Hospice of Cleveland is seeking a skilled Speech Therapist to join our Home Health team on a PRN basis. This is an excellent opportunity for a motivated and compassionate professional to make a meaningful difference in the lives of our patients.

Job Summary:

The Speech Therapist will be responsible for providing high-quality speech therapy services to clients in their homes, in accordance with the Plan of Care and Agency policies and procedures. This role requires a strong understanding of speech therapy principles and practices, as well as excellent communication and interpersonal skills.

Responsibilities:
  • Conduct comprehensive speech therapy assessments and diagnostic tests to identify client needs and develop effective treatment plans.
  • Provide individualized speech therapy sessions to clients, using evidence-based practices and techniques.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Document client progress and outcomes, using standardized assessment tools and outcome reports.
  • Participate in care conferences and collaboration meetings to discuss client care plans and outcomes.
Requirements:
  • Graduate of a Speech Therapy program.
  • Current registration or license as a Speech Therapist, as required by the state of practice.
  • Minimum of one (1) year experience in a healthcare facility or equivalent experience preferred.
  • Excellent written and verbal communication skills, as well as strong interpersonal skills.
  • Licensed driver with automobile insured in accordance with state/Agency requirements.
Working Conditions:

This role requires a full range of body motion, including the handling and lifting of patients. Manual and finger dexterity, as well as eye-hand coordination, are essential. The Speech Therapist may be required to work under stressful conditions or irregular hours, and may be exposed to communicable diseases.
