Job Title: Home Health Speech Therapist

Advanced Home Health and Hospice of Cleveland is seeking a skilled Speech Therapist to join our Home Health team on a PRN basis. This is an excellent opportunity for a motivated and compassionate professional to make a difference in the lives of our patients.

Job Summary:

The Speech Therapist will be responsible for providing high-quality speech therapy services to patients in their homes, in accordance with their Plan of Care and Agency policies and procedures. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.

Responsibilities:
  • Conduct comprehensive assessments and develop individualized treatment plans for patients
  • Provide speech therapy services to patients in their homes, including assessment, treatment, and education
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination
  • Document patient progress and outcomes in accordance with Agency guidelines
  • Participate in care conferences and collaboration meetings to ensure effective communication and coordination of care
Requirements:
  • Graduate of a Speech Therapy program
  • Current registration or license as a Speech Therapist in the state of practice
  • Minimum of one (1) year experience in a healthcare facility or equivalent experience
  • Excellent written and verbal communication skills, with the ability to work effectively with patients, families, and healthcare teams
  • CPR certification
Working Conditions:

The Speech Therapist will work in a fast-paced, dynamic environment, with frequent travel to patients' homes. The ideal candidate will possess excellent time management and organizational skills, with the ability to prioritize tasks and manage multiple responsibilities.
