Job Title: Case Manager

Job Type: Contract

Salary: Starting at $19 per hour

Location: One location

Anew Behavioral Health is a mental health agency seeking a professional and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for coordinating and providing care that is safe, timely, effective, efficient, and client-centered.

Key Responsibilities:

  • Coordinate and provide care that is safe, timely, effective, efficient, and client-centered.
  • Handle case assignments, draft service plans, review case progress.
  • Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options.
  • Work with therapists, clients, and client family members to ensure that client needs are met.
  • Offer facilitation of achieving personal independence and assistance with development of daily living skills.

Requirements:

  • Background check required
  • CPR Certified preferred
  • Behavioral health experience (1 year preferred but not required)
  • NPI number/Ohio Medicaid ID preferred but will be required to start. (We offer instructions on how to apply)
  • High School Diploma minimum, College degree in Human Services preferred
  • Experience working with Children and Adults, preferred
